Care and Recovery Coordination

Care and Recovery Coordination (CRC) is a catchment-based service supporting high-risk clients with problematic alcohol and other drug (AOD) use and other complex needs in the Inner East.

CRC facilitates integrated treatment and care pathways for individuals who require multiple-agency, collaborative care from alcohol and other drug AOD- treatment services, and other health and support services. Experienced CRC coordinators work with clients to set recovery goals and develop treatment plans, and also can assist with submitting referrals and building collaborative service pathways.

Suitable for

Individuals accepted typically present with behaviours or conditions that place them, staff or the community at higher risk (e.g. recently discharged from the Severe Substance Disorder Treatment Act or requiring a long-term supportive service to work towards AOD treatment goals).
